Long Term Care / Nursing Homes Client Services

Long-Term Care Homes (also known as homes for the aged and nursing homes) are subsidized residential facilities that provide care to individuals who are unable to live independently in the community as they require the availability of 24-hour nursing care and supervision within a secure setting. In general, Long-Term Care (LTC) Homes provide typically higher levels of personal care and support than can be provided in the home or offered by Retirement Homes or through Supportive Housing options. Some LTC Homes also offer short-term respite care.”

What is The Process for Applying to Long Term Care Homes?

For all long-term care homes in Ontario, admissions are coordinated through the Champlain Local Health Integration Network (LHIN), which provides in-home health and support services, and coordinates the placement of clients into long-term care homes.

Once you have decided to go forward with your application, the Champlain Local Health Integration Network (LHIN) will assign you a Case Manager to guide you in gathering the medical and personal information required for admission. Your Case Manager may offer help, if you need it, in choosing a home. The Case Manager can give you estimates of wait times for your choice of facility.

In general, once called, families have 24 hours to decide if they will take a space in the home that has become available and have 5 days to move in.

Within Perley Rideau there are 250 designated for qualified Veterans. Veterans Affairs Canada determines the eligibility of each veteran for admission. Please contact Veterans Affairs Canada for further information on the admission process in long-term care.
